Adult Foster Care. In Indiana MENTOR’s Adult Foster Care program—also known as Host Home—adults and seniors with intellectual, developmental and physical disabilities, as well as traumatic brain injuries, live in private family homes with dedicated caregivers who we call Mentors.
The Mentor, in partnership with Indiana MENTOR program staff, medical personnel, the individual's family and others design and implement a Personal Care Plan to meet the needs of each person we serve.

In our Adult Foster Care (AFC) program—also known as Host Home—adults with intellectual and developmental disabilities live in the private family home of their dedicated AFC providers who we call Mentors.
